Dear Colleagues,

I hope this newsletter finds you well.



As a Federation, we continue to strive to support practices during these challenging times faced by our NHS. As many of you know, we have been engaging with pharmaceutical companies to deliver several projects. Currently, we are exploring the potential for a Spirometry HUB using our own practice nurses. We are also in active discussions regarding a Heart Failure project. I have had several meetings with the Trust, which is supportive of this initiative, and a regional model is being developed. This project would potentially allow some of our GPPs to develop project-based skills, which unfortunately have not materialized thus far, but would be backfilled.


We have continued to progress with the recruitment of our GPPs, which will allow us to increase sessions to help with backfill and other work for practices. Work continues on the GPP plan and development regionally. Now that we have the restoration of government, we will try to push this forward.


Our MDTs continue to work tirelessly in practices, as do our GPPs. I would like to take this opportunity to thank them for all their hard work. An example of their success is demonstrated by David McAuley's recognition at the Nurse of The Year event, where he won a prize and had nominations in two categories. We look forward to completing our MDT service as the next stage of the MDT rollout receives funding.


As mentioned previously, elective care work has been reduced despite our best efforts to advocate for its importance. It is hugely disappointing for those involved, especially given that it affects a small, overperforming part of the NHS. We will continue to push for the restoration of sessions as soon as possible. You may have received an email informing you that the vasectomy service will resume at the end of June and will be taking new referrals.

Celebrating Double Award Success








In the last quarter we have been following the success of Senior Mental health Practitioner, David McAuley, who was nominated in two categories in the prestigious RCN nurse of the year awards:  the Primary Care Nurse Award and the Digital Innovation in Nursing Award.

 

Davids nomination focused upon his role in developing The Causeway Emotional Health and Well-Being Guide, a website, dedicated to fostering mental health and well-being within our community. 

 



The RCN NI Northern Ireland Nurse of the Year Awards ceremony was held at the Culloden hotel on 13th June 2024 and the mental health practitioner team were all excited to able to watch and support David on live stream.

 

The first category up was Primary Care Nurse and were delighted as Davids inspiring dedication and hard work was recognised as he was announced as the runner-up for the Primary Care Nurse Award!

 

Next category up was Digital Innovation in Nursing Award.

We were all proud and delighted when we heard  the host, Barra Best announce, ‘The inaugural Digital Innovation in Nursing Award goes to David McAuley, primary care mental health practitioner within the Causeway GP Federation, for his role in developing a digital platform promoting high standards of mental health care.’

Congratulations David, a truly well-deserved and brilliant accomplishment.

Your innovative approach is making a real difference!

 

The team are continuing to promote and develop this resource and David McAuley and Louise McKechnie recently attended the MDT Leadership and Learning Event to present the Causeway Emotional Health and Wellbeing Guide sharing this valuable initiative, which we strive to grow, and improve to support our community's mental wellness.


 

In other news, the team continue to face illness and change; Causeway GP Federation is seeking to recruit a new permanent Full Time MDT Senior Mental Health Practitioner.


This comes with the sad news that Louise Connolly will be leaving the team. Louise will be great loss for the mental health team but we would like to congratulate her as she follows her passion in the care and support provided to women and their families who are experiencing mental health problems in pregnancy and after having a baby.


Congratulations Louise in your newly appointed role as Perinatal Mental health Occupational Therapist with the NHSCT specialist perinatal team.

 


Thank you to all the Senior Mental Health Practitioners who, despite team illness and change, continue to be extremely busy and working hard to provide a high standard of accessible, expert service, within the GP Practices in Causeway GP Federation.

MDT - First Contact Physiotherapists

Suzanne Kennedy - Consultant Physiotherapist


The First Contact Physiotherapy team continues to support all the GP practices in the Causeway Federation with their expertise in assessing, diagnosing and managing musculoskeletal conditions.


The staff continue to develop their skills and we wish Adam Wilson and Paula McLister good luck as they are currently undertaking training in injection therapy and are soon to sit their exams. This will add to their repertoire of advanced practice skills. All FCP non-medical prescribers attended a CPD day on analgesia in April to enable them to evidence ongoing learning. They have now completed a comprehensive competency framework and have successfully re-registered as NMP for the next year ahead.


I would like to welcome Shauna Cloughley back from maternity leave and just when the whole team is re-established change is underway again.


It is with regret that Graham Blair is leaving the FCP team but I would like to thank him for all his hard work and wish him all the best in his new role. Recruitment for his post is currently under way.


The Causeway MDT were very proactive in recruiting service users for feedback in the regional 10k Voices Patient Survey and had the highest response rate across all the professions. Formal feedback will be presented in the near future.


I am pleased to report that the QI project on obesity has very promising results with 80% of patients showing a reduction in BMI at 5 weeks. There was an improvement in the levels of physical activity and well-being score and a reduction in waist circumference demonstrated as well. A longer term follow up period will continue to see if these improvements are maintained.


Progress is continuing on the development of a SOP to facilitate the issue of Med 3 Fit notes in primary care which should hopefully come into effect in the near future.


Self-referral for physiotherapy has been launched in the Northern Trust to assist GP practices that are waiting to be allocated an FCP but is available for all patients over the age of 16. Criteria and forms can be accessed via the link or QR code below:

 

 

MDT - Social Work

Elizabeth Craig - Social Work Lead

 

We are delighted to announce that we are recruiting 2 new Social Work Assistants; interviews will hopefully take place in early July 2024.  These are both 3 day posts but will be of great assistance in practices.

We are also recruiting a full-time administrator shared between the MDT Social Work Lead and the First Contact Physiotherapy Lead. 

 

Unfortunately, we are losing Kerry Ferson, one of our part time Senior Social Workers in Lodge Health, and we wish her all the best as she goes to work on a full-time basis for the Children and Adolescent Mental Health Service in Ballymoney. Kerry will be sorely missed but we hope we can recruit another Senior Social Work Practitioner into this role quickly.

 

Meanwhile, there continues to be much great work on-going across the practice staff.

 

The Carers Support Group in Coleraine continues to grow and now is extended to all patients across the Causeway GP Federation. There is a monthly support group which is regularly updated on information on local resources/services/support available.

 

This work has included a very successful 6-week Mindful Carers Wellbeing & Resilience course by Action Mental Health which took place in April 2024 and received very positive feedback. There has also been a 6-week Positive Minds through Art Wellbeing programme in Portstewart in partnership with Meraki Art NI, and a 6-week CBT programme on Living Life to the Full for Male careers by AWARE NI.  On 25th April 2024, Clinical Psychologist, Dr Shane Martin from Sligo, provided training to this group on resilience and emotional wellbeing. The group numbers are now well above 100 participants and activities have been extremely beneficial. Throughout the year there are ongoing events including an annual Carer’s Fair and A Carer’s Rights Day.

 

We are grateful to Marian Doherty (Killowen and Riverside), who remains active on the Carers Pathway Steering Group for the Northern Trust, and after helping run a very successful Carer Information Fair held in the Lodge Hotel last year which had 64 exhibitors, is planning a similar event as part of carers week this year.

 

Additionally Causeway MDT Social Work are co-running an event on 12th June with their WHSCT colleagues, the Loneliness Network, NHSCT Health and Wellbeing, Causeway Volunteers and Translink where a ‘Chatty Carriage’ is being provided to allow those who are socially isolated in the community to meet and connect with others.  The Causeway participants will travel to Derry/ Londonderry on the train and the Derry/ Londonderry participants will come to Coleraine. There will be over 200 participants involved and the event is part of Loneliness Week.

 

We are very grateful to Marian Doherty, (Riverside and Killowen) and Terry White, (Mountsandel) for organising and facilitating this initiative.

 

Anna Nicholl, (Lodge) and Elizabeth Craig, Social Work Lead,  together with the Causeway Coast and Glens Borough Council, Acacia Path and GP Federation hosted an information event for those seeking asylum in the Causeway locality, there were around 40 participants who learned about health care, housing, policing issues, drug awareness, medication, physiotherapy and benefits advice.

 

We are starting this year’s seed funding process and encourage you to discuss with your practice MDT staff if you have identified areas of need in your practice that might lend itself to project work. Some projects that have previously benefited from seed funding include


  • HUG-(Helping Us Grieve) support group- continues to go from strength to strength with a 20% increase of participants last year-feedback is positive and connections are well established with participants helping and supporting each other. Hug have been involved in a number of initiatives are taking part in the ‘Chatty Carriage’ coming up.


  • AWARE(mental health project with Ashes To Gold)- 6 week programme with practical workshop sessions, horticulture and Mindfulness-target group consisted of low level mental health, social anxiety and carer stress-we have received fantastic feedback and participants are now taking ownership of the group allowing MDT staff to gradually step back, perfectly demonstrating the seed funding concept.  Another initiative is being launched in September 2024.


  • M Power (Menopause events)-An event was held on Tuesday 5thMarch at Kelly’s Complex in Portrush (awareness/information evening) which  included presentations from GP, Gynaecology and Mental Health.  The event was extremely successful with around 100 participants and very positive evaluations.  A further Menopause Clinic is being considered and possible collaboration with a local gym.  Thanks to Terry White and Cathy Watson (Mountsandel) for developing these projects.


  • Connect Wednesday- a social café which operates from Terrace Row Church each Wednesday (anyone welcome) includes art therapy, board games and a "lunch day” on the first Wednesday of each month.  This group continues to grow. 

The Men’s Group in Garvagh run by Carla McLaughlin and Paul Johnston continues to go from strength to strength with participants involved in therapy via woodwork.  A short film has been made by a local filmmaker of the benefit of the group and has been nominated for awards in Belfast and Galway. We have also ongoing walking groups and wellbeing groups across the Causeway Federation addressing mental health, social isolation, loneliness and poverty. The Craft and Walking Groups run by Lyndsay Peden and Sara Jane O’Donnell in Ballymoney have been very successful and are now being taken over by the participants involved. 


Additionally Reflecting Families and the Neurodiversity Groups established and facilitated by Kerry Ferson (Lodge) and Sara Jane O’Donnell (Ballymoney Family Practice) respectively have run very successful programmes for children and parents facing challenging family circumstances.  Anna Nicholl (Lodge) continues to successfully support several Ukrainian Groups which assist people across the age range. Claire Reid (Lodge) continues to assistant with an Equine Therapy Group in conjunction with the RDA to assist children who struggle socially and Joe Mulholland and Paul Johnston (Rasharkin) have successfully established a ‘Warm Hub’ and ‘Luncheon Club’ in Rasharkin.


We encourage any practice staff to discuss with the social work team if you think any patients would benefit from linking in with these projects or if there is a need to establish something similar in your area.


 








Donna Campbell and Mary McBride - Lead General Practice Pharmacists


Since my last update, I am delighted to announce that Mary McBride has joined the Causeway Federation team as my co-lead! Mary has a wide range of experience in primary care, community pharmacy, and SPPG, and has a special interest in pain management. We have also been extremely fortunate to recruit three new relief GPPs: Cara Murdock, Kathryn Kane, and Gemma Clohessy, who will help provide cover for maternity leave, long-term sickness, and backfill for some medicines optimisation projects, which we hope to start rolling out across the federation in the autumn.


Emma McDowell, the pain management deprescribing pharmacist from the Trust, has been supporting GP surgeries with pregabalin deprescribing since January 2024. She is covering maternity leave for Kirsty McClintock, who will be back this summer. Practices with high rates of prescribing are being targeted first, and the short- to medium-term plan is to roll out the programme to all GP surgeries within the Causeway Federation. Emma details her work to date below.


“The programme started with pregabalin as it has been removed from the NI formulary for chronic pain, but it is anticipated that the service will expand to gabapentinoids, and then opioids, as it progresses.


Overall, the programme has gone smoothly to date! Patients are often not keen or are scared to stop pain medicines, especially if they have been on them a long time or are still in pain. Counselling the patient on the following points is usually enough to get the patient on board with starting the reduction process, with an emphasis on taking it one step at a time, with regular reviews available should the patient wish:


  • The fact that the medicine does not have good evidence for working in the long term.

  • Missing a dose and feeling terrible does not mean the medicine is working; this is usually a sign of withdrawal.

  • Advice on the side effects and long-term harms, and personalising this to the patient.


There have been significant numbers of patients who have gone through the deprescribing process with no issues. We always remember the difficult or resistant patients when deprescribing, but it’s important to remember that many patients do not understand that the risks of these medicines are often worse than the perceived benefit. Once this is explained, they are usually very willing to work with you to reduce and stop. Providing support and advice throughout the process has a major impact on successful deprescribing, and pharmacists are well-placed to provide this.”

A booklet of pain management resources, which can be used by healthcare professionals or shared with patients, has been developed in collaboration with colleagues from the Pain Management Programme. It is currently hosted on the excellent Causeway Emotional Health and Wellbeing Guide, within the Pain and Long-Term Health Condition Management tile. We hope to share it more widely in the near future. The booklet contains information on available community pain management programmes, apps to help with chronic pain, signposting to resources for exercise and movement, and much more—all in one place. Sharing this information with patients earlier in their pain journey will hopefully impact their understanding of pain and how to manage it long-term.

PBL continues to go from strength to strength with many facets of training across the various disciplines within primary care.


Subject to funding, we hope to run a "Cultural Confidence" workshop through the Law Society, titled "Liaising and Working with People Seeking Asylum in Northern Ireland," in September.


This will be relevant to all disciplines. We are thankful to our pharma colleagues who continue to support us and, as always, ask that you engage with them where possible. It is great to see such a turnout at our PBL events, which always provide a wonderful networking opportunity for colleagues.

Colleagues,

I wanted to update you regarding the current ‘state of play’ relating to GP Elective Care Services .


Most will be aware of the farce around vasectomy, with service initially being suspended then progressing to closure. Fortunately the Health Minister stepped in at the very last minute and we are now in the position of recurrent funding being approved for the vasectomy service. All future vasectomies will now be carried out within Primary Care apart from those requiring a general anaesthetic or have other complicating factors.


Due to the suspension of the service we have built up longer waiting times than we would be happy with. Staff within GPECS / FSU and our Host practices have worked tirelessly in the last few weeks to relaunch the vasectomy service. We plan to develop the service over the next month’s / years in order to achieve more acceptable waiting times.


CCG will be reopened to allow GPs to again refer patients into the service on 29th June 2024.


Absolute Exclusion Criteria 

          -  Weight >160kg 

           - Implanted cardiac device or pacemaker

            - Uncontrolled diabetes HBA1C > 69mmol/mol


Relative contraindications –Gpes will assess and decide if surgery is possible ]

-          BMI > 35

-          DOAC/ Dual anti platelets

-          Previous scrotal Surgery / Hydrocele

-          Immunosuppressant’s

-          Severe anxiety  


Our other services – Dermatology, MSK, Skin Surgery and Gynaecology remain at under 50% of their previous activity. At present there is no budget allocation to any department within Trust or Primary Care and thus we are still optimistic that recurrent funding may be allocated to these services.

We are delighted to have almost all our Emis Practices joined up to the FSU contract for AccuRx. A platform to connect and communicate to patients which has a number of modules to help support Practices. Accurx | The simple way to communicate about patient care.

NFSU hold the contract alongside Eastern, Western and Southern Federation Support Units, those who are in the contract provides practices with a large, reduced discount than purchasing this software alone.

As some of you may be aware, the East Antrim Federation, NFSU and the Northern Regional College collaborated together in 23/24 to launch the new Level 2 Primary Care Medical Administration course in Newtownabbey for EA Practices. Building upon the success of this course, we are delighted to share that NRC has expanded this course to the new NRC Coleraine campus in 24/25 and Causeway Federation Practices.  Primary Care Medical Administration - Level 2 Diploma - Social Care & Early Years at Coleraine - NRC

 

This course was designed by Practice Managers in EA to Enable our local GP Practices & increase future careers in General Practice. This initiative is bringing the health and education sectors together to support General Practice and represents a significant step to help meet the growing demand for GP administrators within our local GP Practices.   

 

This course runs from September 24 to June 25 with the student being 18+ and a minimum of 4 GCSEs. The student will study in NRC 1 day per week and will be in placement in your practice 1 day per week (7.5 hours). After the course completes, there will be a list of trained students who may be available for recruiting.  

This is subject to small changes based on feedback from this years course. You will not be required to employ or pay the student, they will be on placement as such.

 

Finer details are yet to be confirmed. In order for the Federation to look at this further, we need you to complete a quick survey to gather interest in Causeway. Ebrington

You will be aware we have been in touch with Ebrington Medical to achieve economies of scale cost savings. They have established a “buying group” which Practices can avail of further discounts. Ebrington are currently uploading Practice details to their system and will be in touch the minute this is complete.


Clinical Waste

We understand the challenges by the rise and increasing costs of clinical waste disposal within GP practices in Northern Ireland. In response to concerns raised by GPs and Practice Managers, the Federation has proactively engaged in discussions to explore avenues for cost efficiencies on your behalf. The Northern Federation alongside Western, Southern, and Eastern Federations have initiated discussions with Stericycle, headquartered in Ballymena, to negotiate discounted rates for our members. Several practices across the region already partnered with Stericycle have provided positive feedback on the quality of service they've experienced. Practices can transition seamlessly once their current contracts agreements expire. The proposed pricing schedule demonstrates that as more practices join the contract, the discounts available to all members will increase substantially. Further details are available in the attached proposal and costing schedule. The goal is to have >100 practices across the region in the contract reaching up to 25% discount overall!! The contract will be held with the individual Practice and the FSU has come together for discount purposes.

NFSU Meeting Rooms

NFSU based at Galgorm Castle has two meeting rooms FOC which can be used individually or opened to one large room. They have video conference cameras/ TVs which can be connected to laptops. We have coffee and tea making facilities and local catering companies nearby. We welcome any practice who wishes to use these rooms which are free of charge for any training/ meetings. NORTHERN IRELAND GP Practice Management Handbook

 

The Practice Managers Advisory Group within Eastern FSU has been working to develop a GP Practice Management Handbook, the first of its kind in Northern Ireland!

This project was undertaken after a need was identified to have a comprehensive reference guide, specific to Northern Ireland, primarily for new practice managers. It is hoped this handbook will aid new Practice Managers (and other staff) with induction into the Primary Care setting and be a supportive resource for existing managers in their day-to-day business operations.

The document has been designed to be used as a digital document as it has live links throughout but as things frequently change in general practice this handbook will be updated on a yearly basis.
